Immunization Records

Students entering our school for the first time are to bring a record of four DPT (or DT), three Polio, one Measles (10 day) immunizations and immunization records for mumps and rubellas. The month and year of these immunizations are to be furnished to the school office before the student can attend classes. While not yet a State of California regulation, a tuberculosis test, a hepatitis series, and a second measles immunizaiton is strongly recommended. Please contact the school nurse for information.

There is a new state law requiring all students entering 7th grade to be immunized against hepatitis B. Although a notice was sent home earlier, we thought it was important to remind you that children who have not completed their hepatitis B shots before the first day of 7th grade must be excluded from school. We are required to see your child's Immunization Record as proof of immunization. You may present this in person, fax or mail a copy to Rio Hondo Prep, P.O. Box 662080, Arcadia, CA, 91066-2080 (Fax 626-442-1113).

RHP will not issue any medications (including Tylenol) unless written permission is provided by the parent. Medications are to be marked with the student's name, directions for use, be accompanied by a signed permission note turned in to the nurse's office. RHP cannot be responsible for reminding students to take their medications, but the nurse will be happy to assist in this matter.
